The "Jaw in a Day" is a revolutionary Surgical Technique that allows same day dental rehabilitation for patients with head and cancer, ORN and benign tumors. It was first described by Dr. David Hirsch in 2013, became quite popular. This was certainly the most rewarding and exciting case of 2025 that I performed.

The "JIAD" is a cutting-edge procedure integrating tumor resection, jaw reconstruction, and dental rehabilitation into a single surgery. This innovative approach is certainly transforming the treatment of benign jaw tumors in our practices.

We start by virtual surgical planning to plan for resection of the biopsy proven tumor while preserving vital anatomy, precise placement of implants on the fibula free flap while the leg is still attached and designing the restoration as well.
Preoperative virtual planning enables same-day fibula resection and dental implant placement and provisionalization with a milled PMMA temp with some chairside conversion .
The advantages of JIAD are reduced treatment time, improved aesthetics and function and just one streamlined surgical recovery.

The biggest challenges are lack of patient and doctor awareness, technical complexity requiring interdisciplinary collaboration (Head and Neck, OMFS, Prosthodontics), lengthy surgery (10-15 hours) and lastly higher upfront costs (Plates, Implants, Restoration, Insurance issues). Additionally some hospitals don’t allow lab technicians or dentists in the OR so as a surgeon in need to be able to pick up and convert leg-side.

Ideal Candidates are healthy young patients with benign tumors due to predictable resection margins, and favorable reconstructive anatomy without radiation sequela. Studies demonstrate excellent functional and aesthetic outcomes, high implant success rates, and significantly improved quality of life.
My patient is a 19 year old female with a rapidly enlarging large mandibular ameloblastoma that was seen in our clinic that was planned for resection and immediate reconstruction with a Plastic Surgeon and Prosthodontist.
In the past with a large tumor like this patient, they would undergo an initial mandibular resection with delayed or immediate reconstruction using a staged hip or fibula bone graft. After appropriate healing, they would undergo placement of dental implants, depending on whether they had received radiation or not. Many centers continue to place a fibula without implants for these patients.
Most patients when asked about teeth were often told by the treating physician and team that they potentially may not have teeth again. Studies have shown that Immediate dental rehabilitation restores form and occlusion, enhancing the patient's appearance and ability to eat, speak, and chew.
We successfully planned the case and executed it flawlessly thanks to Stryker CMF an Nobel Biocare. The teeth were perfect, and occlusion was verified with the stealth articulator developed , it needed to be slightly adjusted after the pickup and delivery, but it was a home run. We knew the standard temporary copings wouldn’t be tall enough so we had the lab mill some extra tall impression copings ahead of time. Patient is now 3 months post op and had the smoothest recovery compared to many similar patients I’ve treated in the past. We will perform our digital workflow for fabrication of the final in a few weeks.
My pearl is that thanks to the "Jaw in a Day" surgery our patients with benign tumors have enhanced outcomes as they enjoy a reduced treatment time, avoid multiple surgeries and lengthy restorative appointments. The VSP and preop planning allow improved aesthetics and improved function as opposed to the old way of waiting and staging.
