Wisdom teeth don’t always require extraction—but asymptomatic doesn’t necessarily mean disease-free.

In this case, the patient presents with a well-defined radiolucent osseous lesion involving the apical region of #17 and #18, along with external resorption of the distal root of #30.

The case highlights the importance of proactive third-molar evaluation. Changes in surrounding bone and damage to adjacent teeth can develop even in the absence of pain or other symptoms.
