Patient presented with a crown fracture on tooth #5, previously endodontically treated. Unfortunately, the patient never returned for the final crown, and over time, functional forces led to structural failure.

Diagnosed as a hopeless tooth due to fracture.

Treatment performed:
• Atraumatic extraction of #5
•Osteotomy through the socket with a palatal trajectory
• Immediate implant placement
• Simultaneous bone grafting
• Placement of a 4.2 x 13 mm implant (Implant Direct)
• Fabrication of a custom healing abutment using a stock abutment and flowable composite which will help preserving the natural gingival architecture and set the foundation for an optimal final restoration.
